Day programs for adults with disabilities provide a place for older adults as well as younger adults to be active in the community, outside their homes. They can also socialize with their peers while still having access to health and personal care services. West Mont work/day programs also provide ample opportunities for people to stay involved in the community while still receiving the care and supervision that they need on a daily basis.
Supported Living is an all-encompassing service delivery model which takes into account the desires, goals, strengths, abilities, needs, health, safety, and age of an individual living on their own (apartment, townhouse, or other residential settings) but needing support. Supported living is a long-term service which can change or vary in scope, duration, intensity, or location as the needs and preferences of the individual changes over time. Supported living services are designed to promote successful community living, maximizing independence and prevention of psychiatric hospitalization.
West Mont provides assistance and training with personal care, medication management, meal planning, cooking, shopping, and social /community activities in safe and secure group homes and apartments. West Mont staff is on site 24-hours a day 7-days a week.
West Mont offers a variety of vocational and employment choices based on an individual’s interests, abilities, skills, and available openings. Our trained staff provides clients with the tools necessary to develop basic vocational and life skills.
